Entrance assessments are required for grades 2nd - 12th.
Prepare for students to be on campus for the entire school day. Students should dressed in khaki bottoms, and collared polo shirt for assessment day if testing during the school year.
To access the online application create a family account in myschoolworx, and upload all required documents (see step 4).
Be sure to write down the login and password for future usage.
Log into your myschoolworx account and upload all required documents:
Copy of Birth Certificate
Behavior Report (for example: the OCPS report is called “Student Detail Profile”).
Immunization Form DH 680
